1. You can stay in Sanur and do a day trip to Nusa Penida, or it's also possible to stay a couple days on Penida island to do more sightseeing there. I think it's best to spend at least two days and one night in Penida if you can, but if you don't have time for that then a day trip is still worthwhile. You can read my Nusa Penida travel guide for some ideas of the best things to do on the island.
2. For Ubud, a good midrange option is SenS Hotel, which is located right near the town center. Or a good budget hotel is Ubud Glassy Villa. For Sanur, a good hotel option is Parigata Resorts or Puri Hiromi. For Nusa Penida, a good hotel option is The Sagara. For Uluwatu, a good family option is Bingin Bienvenue or similar.
3. Sure, if you have an extra day you could visit Lempuyang temple and some other sights on a day trip from Ubud or Sanur. Here are some more ideas of things to do in East Bali. I hope this helps!

1. For Nusa Penida, should we book hotel in Sanur or Nusa penida itself?
2. I will be travelling with my spouse and 2 yr old baby. Can you provide your recomendation about hotel names, as I am following your above itenary.
3. I will be having 11 days, so where hsould I go 1 more day? WIll I be able to cover Lempuyang tempel near Amed area?
